Extend-A-Family has been the regional administrator of the Passport Program since 2006 and currently oversees the funding of over 3700 individuals across the South East Region. Our agency and team of staff are dedicated to providing individuals with disabilities and their families helpful access to their funding.

Who is the Person Managing Funds?
The PMF may be the recipient of the funding (client) who can choose to accept the funding and the terms of the agreement. They will sign as the client and the PMF.
If the recipient requires support to accept the terms of the agreement then the client may designate a person to be the PMF and accept the funding and terms on their behalf (Ex. Parent, sibling, etc..)
If the recipient has an active legal substitute decision maker (Power of Attorney of Finances) or a Public Guardian & Trustee they will sign as the PMF.
The PMF will determine how the funding will be administered.
How Can I Use My Funding?
COMMUNITY PARTICIPATION SUPPORTS
Hiring a worker to help a person participate in social and recreational activities, employment and pre-employment activities, volunteer activities and other supports.
CAREGIVER RESPITE
Hiring a support worker to provide care-giver relief to the person’s primary caregiver; this can involve in-home and out of home activities.
ACTIVITIES OF DAILY LIVING
Hiring a worker to support the person with meal preparation, personal hygiene, grooming, dressing and training related to using public transportation, banking and other life skill.
